Eye Examination

  • A maximum benefit of $100 is payable for each eye examination.

Lenses and Frames

  • Eyeglass lenses and frames or contact lenses, excluding sunglasses.
  • A maximum benefit of $300 in any 24-month period.
  • Replacement of existing eyeglasses or contact lenses will be covered after the policy has been in effect for 12 months.
